Sign up for a Services Québec course

In accordance with the agreement between Champlain Regional College for and on behalf of its constituent college Champlain College Saint-Lambert (the “College”) and Services Québec under the formations de perfectionnement program whereby the College can offer training at a reduced cost to the learner,

I consent and understand that in order to benefit from the subsidized rate of 5$ per training hour, the College will collect and share the below personal data to Services Québec.

· Participant: first name, last name, home postal code, city of residence, employment situation (employed, self employed, unemployed), name of their employer, city where the employer is situated, name of the training they took, training hours associated to the course.

I understand the above personal data is required for the College to uphold its agreement with Services Québec and if I remove my said consent mentioned above, I will not be eligible to receive the training at the cost of 5$ per training hour. Shall I wish to remove my consent, I will need to email the College at champlainbusiness@crcmail.net
