Logistics Training Courses (CITT) – Admission Form

In accordance with the agreement between Champlain Regional College for and on behalf of its constituent college, Champlain College Saint-Lambert (the “College”) and CITT as part of the logistics training program leading to “CCLP” certification,

I understand and agree that the College may collect and share the following personal data with the CITT.

- Participant: first name, last name, email address, name of training program completed, grades obtained.

I understand that the above personal data is required for the College to comply with its agreement with the CITT and that if I withdraw my consent, I will not be eligible to receive the training. If I wish to withdraw my consent, I must send an email to the College at champlainbusiness@crcmail.net.

In accordance with the Act respecting access to documents held by public bodies and the protection of personal information, you have the right to access your personal information as well as the right to rectify this information.
